Go to Bevscountrycottage.com = look for Bev's bear beanie. The one she shows also has a face on it but the hat and ears are like the pic you had.

Hope this helps. If not go to goggle.com and put in free crochet pattern for baby hat with ears; or bear hat and quite a few websites should come up.
