When I first started knitting, I also had problems with holes. As you gain experience knitting, you will be able to figure out where they came from. If you count your stitches and find you have more than you started with, you probably accidentally made a yarn-over (used in lace knitting) or accidentally pulled your yarn tight pulling the "legs" of the stitch onto the needles then purled into each one of them (used in some short row techniques).
If you have fewer stitches than you started with, you probably dropped a stitch. If you tug at the bottom of the hole and it gets bigger, you know you've dropped a stitch. When done intentionally, dropped stitches are used in lace knitting).
